The referral form your GP is a critical means of communication with your specialist about other medical problems, not just your cardiac issues. It is very important to make sure hat your treatments takes these into careful consideration too.
A Current Referral is necessary to receive the Medicare Rebate for the consultation.
A referral from a General Practitioner is usually valid for one year.
A referral from a Medical Specialist is only valid for three months.
You will need a referral from your GP or specialist to make an appointment. Please obtain this prior to making an appointment with us. You can forward your referral via email or fax. Alternatively you can request that your doctor forwards the referral directly to us. Please do this as soon as you make your appointment.
To make your visit as smooth as possible, please complete and return your registration paperwork to us at least one week prior to your appointment.
Our staff can either email you a copy of the patient registration form to complete and return, or you can download it directly from our website. It can be filled electronically and emailed back, or you can print, fill and scan a copy for return by email or fax.
